How can I find Alpha Centauri and Polyphemus?

| I've seen people getting to the Polyphemus, the same planet that was in Avatar, or rather its moon, Pandora. But I don't have that planet in my game. Is it a mod? Or maybe it's some secret?
I'm very sorry if somebody asked that before, but I couldn't find the answer anywhere.

| The planet was removed in the current version (0.97.0), due to real exoplanets found in the system.
